Specifically, is this supposed to happen, and if not...why did it?

I can confirm that I have, indeed, purchased the AE DLC on Steam prior to this, but I'm just wondering why it didn't get authorised when the base license authorised a base copy...seems a tad arbitrary.

Anyone have any ideas why this is happening, and if it's supposed to be happening at all?

Same here and I'd guess that it is suppose to be this way. GOG already stated that the offer is only for specific games. In this case it is Surgeon Simulator 2013 and not Surgeon Simulator 2016 AE. You may have to contact support if you want a true answer however and if you do so it would be nice if you could report back here.
I have AE (just played that Donald Trump thing) and did not receive it here. Just the base is free for grabs it seems. Not a dick move, as we get the game "free", but still a weird decision. Maybe checking DLC's isnt just as easy or maybe it shall come later. Or maybe they want to go and see if anybody buys the DLC here. IDK. The point is, just the base game.
