The game apparently doesn't like HDMI monitors/TV's ?!?? I have a 40" TV hooked up to my machine as monitor and when i start the game i get the pop-up asking me to use 3dfx and when i hit OK, my sreen just goes blank. i've been having similar issues with other old games using 3dfx ... hitting escape or trying to reach the task-manager in order to shut down the game has proven unsuccessful. Therefore i can only restart my PC in order to get back to the desktop.

On a hunch i connected my old 4:3 (1280:1024) monitor to my PC and tried the game again, this time my 40" TV went dark but my 4:3 monitor stayed on. However i cannot move my mouse to the second screen, seeing that the game was developed pre multi-screen desktop setups, that makes sense. But i can still not get back to desktop or play the game.

When i disconnect my TV and only plug in the old screen, the game starts without any problems in fullscreen mode. It doesn't appear to have any configurable setup ingame, so you can either start the game or quit it. Both work fine - however now i have no sound as i have no speakers connected to my machine, due to my using a TV regularily as i use the build-in speakers of the TV.

My question now is: does anyone have an idea how i can force an old 3dfx-title like this to cooperate with a modern TV set or specifically work with a HDMI connected monitor/TV instead of an old-school monitor? I've been trying now for days to find work-arounds based off of other old games i encountered had similar problems with modern components, but to no avail. Also its kinda frustrating having to plug in another screen everytime i wanna play the game :-/

I'm actually playing this on a 32" TV I user as a monitor with HDMI and I've had no problems at all.... It may be a driver issue for you but I cant tell you that for sure
Did you try an HDMI monitor? Or just an HDMI TV?
Most likely cause: your TV can't handle the game's (low) resolution. It might be an unsolvable 'issue' with your TV, or you could try delving into its resolution/scaling settings.
