If I get a medallion piece and die before the next checkpoint do I have to get it again and again (like fight the same boss all over again for the medallion piece) or can I just skip it the next time and just have it from then on?

Yes, you have to defeat its guardians and collect it over and over again, which is particularly nasty with the one before the final boss fights of the wolf world...
