Ran the game but it displayed some weird color issues, so i ticked the boxes, telling the application to run the game in 256 color mode, as well as in 640x480. Then it worked...but...
...is there any way to force the game to accept a higher resolution then that, a hack/patch perhaps? Anything??
I play this game on my netbook and there it doesn't look any where near as low-res, most likely due to the screen being smaller, but that can't be the whole story as there really is a huge difference. On my Win 7 PC system, using a 24" monitor, Sanitarium looks absolutely dreadfully low-res. Looks great on the netbook though.
Any info is appreciated, as i would love to keep playing the game on my larger screen instead of my netbook, but as lovely as the game is, the low-res state of it on my stationary system, is unbearable and a lot of the ability to spot details in the games' surroundings on my netbook, gets lost here.
It doesn't seem like this game has any mods, I have been searching and have yet to find anything.

The only project that will be capable of doing something similar will be the following http://code.google.com/p/asylumengine/ assuming it's gonna come with a scaler.
Since the project will eventually merge with ScummVM, i think your wish might come true, but not soon enough.
Any ideas yet?
You can run the game in windowed mode (http://www.gog.com/en/forum/sanitarium/windowed_mode) and then resize the window freely.
Upd: But it won't solve your problem apparently :-)

it happens to me too on XP. weird broken colors... it also happens to transport tycoon for some reason.
Atl tabbing back to desktop always fixes it (will try to take screenshot to show whether you are suffering the same problem)

While I won't say it's a magic bullet, if you have an nVidia card, forcing FXAA in the nVidia control panel is an option. I'm running a GTX560 on Windows 7, and I have been running FXAA on Games like Baldur's Gate, The Longest Journey and Sanitarium.

It does hide low resolution a fair bit at the expense of a little blurriness. To me it's worth the trade off. After a short while, I no longer see any blurring of the textures, but I never stop seeing jagged edges without FXAA.

If you're running AMD graphics, MLAA is their equivalent to FXAA, but last I read, it was only enabled via in game settings. Not having the hardware for it, I have not done much digging.

I hope this helps you and others with Sanitarium and any other sprite based games.
