I don't understand, when it comes to a technical problem I keep seeing people on the forums(around the web) saying:

User: Have you installed patch 1.3?'

Me: Yes... or... I think I have. I did everything the ReadMe told me... and I still get bugs and glitches and especially with jumps.

Please help me solve this problem.

I have no choice but to save everytime I do manage to jump/climb over a ledge.

EDIT:
I forced V-sync in my Nvidia Control panel and it works... oh, ok. Go me >:I

For: 120 Hz, 144 Hz, 165 Hz and 180 Hz monitors:

nVidia GPU's:
Right click on desktop -> nVidia control panel -> manage 3D settings -> program settings -> Add: Risen -> ...
a) Vertical Synchronization: "Adaptive with half refresh rate"
or
b) Vertical Synchronization: "On" and Max. Frame Rate: "72" (for 144 Hz screens, set the number to Hz/2)

AMD GPU's:
similar to nVidia settings, but only option b) possible.

(The most important thing is that the game should not exceed 90 FPS)
(no extra patches needed)

You helped me, too. The hint should be sticky.
I've got this same problem with an AMD Radeon 5700 XT. I turned on vertical sync in the graphics card settings, ran the game at 60 fps in the game options, but I still clip through the terrain whenever I try to climb a ledge, any ideas?

If VSync don't work, here is the final solution to the problem (1 minute video):

What's on the video, if it gets deleted? How the problem looks like, and the solution. There is a program called "RTSS" (download link on bottom), you can lock the FPS for selected games in it. There's an option "Start with Windows" on the main window, just unselect. Start the program every time you need to lock the FPS in a game.
For Risen: "FrameRate Limit: 60"
(or don't set it beyond 90 FPS)
