Is there a way to play this game in a lower resolution than that of my screen that is 1080x1920? I tried to change the game option with the gog thingy, but it says the game is not installed. I would be happy with 640x480 windowed but if i can have that full screen with those black bars and not stretched all over my screen that would be even better.

Also after installing the us patch i didn't hear some of the "musics" like the ones in the menu. Is that a normal thing?
And if i press F3 i am still stuck with the normal or easy mode, is there somewhere else i can change my difficulty?

Edit:

Finally managed to install correctly the UK to US patch. A simple extract to path doesn't do the trick, you need to extract multiple times the same file to different directories. In my case :

From the data in the zip to
C:\Program Files (x86)\GOG.com\Realms of the Haunting\DATA\DATA
C:\Program Files (x86)\GOG.com\Realms of the Haunting\ROTH\DATA

From the gdv in the zip to
C:\Program Files (x86)\GOG.com\Realms of the Haunting\DATA\GDV

From the m in the zip to
C:\Program Files (x86)\GOG.com\Realms of the Haunting\DATA\M
C:\Program Files (x86)\GOG.com\Realms of the Haunting\ROTH\M

The rest of the files not in folders (.exe , .ini, .dat) to
C:\Program Files (x86)\GOG.com\Realms of the Haunting\DATA
C:\Program Files (x86)\GOG.com\Realms of the Haunting\ROTH

Got the sounds and all, now the resolution, trying to edit dosboxROTH.conf but still either get just a windowed screen and can't have a blacked background with smaller rez in the center.
For now all i changed was

fulldouble=true
output=overlay
aspect=true

And it seems less pixelated, might be just in my head too.

For slightly improved visuals set the following options too:
fullresolution=1920x1080
scaler=hq3x

Note that fullresolution can be set to any resolution you want.
