The trailer https://www.youtube.com/watch?v=USWwNzE6R7s&feature=emb_logo says Free* (note the astrisk) DLC in the GOG trailer, however it still has a cost to it.

If you already owned Pathologic 2, the DLC was free.

So does this mean that if I buy pathologic 2 now, I have to pay for the DLC separately?

Yes. It doesn't add anything to the main game as it's a self-contained DLC, but it's quite good.
