Every time I get the "Game Over" screen, the game crashes immediately, but that's only when I'm using the software rendering (640x480) in the options. It works perfectly when using the other method, but I prefer this aesthetic of the game. Anybody experiencing this or knows how to fix it?

Same. Would love a fix for this. It's my understanding that the software renderer mode fixes the effects that can't be rendered properly in the other renderer and would prefer the game look truer to the original. I guess it's just a git gud situation until there's a solution?
Yeah, there seems to be a problem with GOGs ddraw wrapper.
Solution: Use dgvoodoo instead: http://www.dege.freeweb.hu/dgVoodoo2/dgVoodoo2.html

Works flawless for me (at least with dgVoodoo v2.64).

Seems they fixed it. I got a small update yesterday, and today I died twice and no crashing!
yes, software mode crashes have been fixed in an update yesterday
