Does this game support DS4? I edited the key config but it does not work in-game whatsoever.

No DS4 support, GOG's support page for the game even says as much. You're gonna have to install MGS2 V's Fix.

for me it worked even without the fan patch but you have to configure the controller.
with v's patch it's easier
