I don't have this GOG version (I still have my CD version, when it was called Metal Gear Solid Integral...).

All those issues are marked as fixed in the GOG version already and the issue is in the gog version still. Hope this either gets fixed officially or someone in the community fixes it.

Edit: Haven't looked at the widescreen fix because I don't want to artificially stretch the image I'm looking for the original 4:3 with black bars on the sides.

Seems like gog is monitoring the forums and trying to fix things so it might happen.
Do the guide from Manu270891, change resolution in mgs1w.ini to "640x480", change in dgvoodoocpl.exe fullscreen scaling to "stretched, 4:3 aspect ratio" in general tab, in directx tab change resolution to "unforced", texturing filtering to "force point sampled", antialiasing to "off", tick in miscellanneus "force vsync" and untick "dgvoodoo watermark".

In the dgVoodoo.conf the dgvoodoocpl.exe program created in the game's directory inside in generalext change resampling from "bilinear" to "pointsampled". And then you get everything pixelated (textures and resolution) like in an emulator without black bar on bottom and with 4:3 black bars.
