Hello, and Happy New Year.

Please note that I'm affiliated with neither GOG nor the publisher, plus I don't have the game, so the most I can tell you regarding your issue is:

1. If you're using the standalone installer, there should be a two-piece separate installer (an exe and bin file) for the Italian language which you need to download; you should have a language selector on the game card in your library (https://www.gog.com/account), and you need to pick Italian so you can download the right installer.

2. If you installed the game using the GOG Galaxy client, I'm afraid I can't help you as I don't know how it handles these things, so you should contact GOG Support.
