Would be nice if anyone could help out.

I´ve tried configuring a joystick, throttle and pedals for this sim.
With actual hardware I mean a not so actual Microsoft Sidewinder 2, X-55 throttle and MFG pedals.

The only thing it seems to recognize is the Microsoft Sidewinder (only as some kind of stick and not as MS Sidewinder) but even with this stick the sim can´t properly recognize the different axis.

Am I stuck with mouse / keyboard or is there a way to get stick / throttle and pedals working?

Look into the joystick section of the .conf, you'll probably need to edit timed=false and swap34=true.

I am kinda late with a reply but thanks for your input - unfortunately this didnt solve my problem.
I don't known if what you like if even possible. Dosbox is very limited with Joystick emulation.

Best ask for help in vogons dosbox section. Those guys are experts making old games to run in modern hardware.

Everything worked perfectly for me, however I have an old copy of the original game CD. I have not tried the GOG version (even though I have it).
I installed the original CD DOS version of Hind in Dosbox 0.74, in Windows 7 64 bit. I remember that I tried to install the game directly in Windows 7 and it worked, but I could not get the joystick work at all, so I sticked with Dosbox.
I used Logitech Extreme 3d Pro joystick. The actual hardware doesn't matter as soon as Dosbox recognizes it.

Edit dosbox.conf like follows:

joysticktype=4axis
timed=false
autofire=false
swap34=true
buttonwrap=false

When you start the game, go to preferences and configure as per attached screenshot.
