I have added an image for a game poster for a steam game that had a messed up one. I'm not sure at what point it stopped working, but it now is blank and when I go to edit it says "Could not load the image" but does not allow me to fix it, so now it is just stuck blank.
I am also having this exact same problem. Disconnecting and reconnecting a platform doesn't fix the problem either.

To add to this, all of my custom images revert back to their original image if I disconnect and reconnect a platform. I'm just happy I didn't customise more than a few games before discovering this problem.

Same problem here. I changed a bunch of DVD covers and now I just can't see the image and there's no way to revert the change.

The same happens here. I notice that all the images that have reverted to default, including the "broken" ones, are from Steam. I have a custom image for King's Bounty installed from GOG, and that still works.
Similar problem here. Nearly all custom images for Steam games have reverted to default. There are also a few Steam games that show no image at all (IIRC, these had no default image). For games with images, it *is* possible to set a custom image again; for the few "blank" games it is not possible to set any images.

hopping in here to follow this, same issue. custom icon game now blank, no option to change it

is there a local db/config where this is saved that can be edited or something?
Just experienced this, too. Can't apply a new image, nor can I delete the old one.
I had the same issue and the only fix was to remove the integration plugin, wait 15-30 minutes until GOG deletes my data from their servers, then re-add the plugin.
I have the same issue. Hoping we get a reset option to remove any custom images.

I have this issue too, and I managed to brute force a workaround (without having to reinstall GOG Galaxy).

1. Open the page for the game with the broken cover art (so you see the game background, screen shots , ect)
2. Click the slider icon at the top of the page and select Edit.
3. Select Media (Should show the Title image space with the “Could not load the image” message)

Now here's where it gets tricky...

4. Open the GOG Galaxy webcache directory ( Unusually C:\ProgramData\GOG.com\Galaxy\webcache\[Big old number])
5. Select the directory of the platform that the messed up game is from (for example Steam)
6. Locate the App ID for the game (may need to trial and error to find it, or if it's a Steam game go to the store page of the game and the App ID will be the number in the URL. For example, 447150 for the game Ape Out)
7. Delete the App ID directory for the game.
8. Alt+Tab back to the Edit Media widow of the game in GOG Galaxy.
9. After a moment or two the “Could not load the image” message is replaced with a broken image icon and you can now add a new image. (if the message doesn't go away after a while try Alt+Tabbing the Game page and then back to the Edit Widow).

The timing of this is finicky and if you navigate way from the Edit window when the broken image icon is present without adding a new image (or it just doesn't work) you'll have to delete the game directory again and retry.

There may be easier ways of fixing this, but this is the only thing I figured out so far.

I am having the same issue, now way to add or even restore my boxarts!
For games I have manually added I fixed it by removing and then adding them again, however for GOG games there is no way for me to restore the boxart.
