Vnlr: Painkiller looks like it might be fun too, can anyone comment on that one?
For some reason, that one didn't work for me at all. There's some crazy enemies, I shoot them, more enemies spawn, I shoot them too ... it should be fun per definition, but it just felt too vanilla.

Other shooters which I didn't like:

CHRONICLES OF RIDDICK had pretty graphic, but feelt way to scripted and console-like.
SERIOUS SAM is a game I just never got into, but I may give it another spin.
ROGUE TROOPER didn't impress me, and is in 3rd person perspective.
SHADOW WARRIOR (2013) was plenty arcade-like, but I couldn't really get into it.
BLOOD is one of the first games I bought here - but sadly, it requres skills, and I suck.

Shooters I love:

F.E.A.R is a gritty combination of nailguns, bullet time and j-horror cut-scenes. (but okay, you already played that one)
TOWER OF GUNS was okay. I liked the idea of a Roguelike-inspired shooter. A bit casual, though.
Rise of the Triad (2013) is plenty fun!
Shadow Warrior Remake, has regenerate health (although you can make no use of it if you don't want to) and no stealth, but is one of the more fun FPS I played recently and one of the main weapons is the Katana, which allow you to cut and slice enemies in all shape and forms.
The Serious Sam games, Shadow Warrior 2013 and Bulletstorm are the best options .

1) The Suffering: Prison is hell. (from zoo digital publishing version, other is full of bugs)

One of the FINEST shooters out there, with a big variety of weapons and grenades/flashbombs/tnt. Flashlight. Monsters. Horror. Survival. Moral choices with consequences. Monster transformation. What else could you possibly ask for?!?

2) Call of Cthulhu: Dark Corners of the earth.

Little buggy, but great lore, good scares/goosebumps, very creepy, extremely innovative (gameplay and features wise).

3) Then there is F.E.A.R. of course! GoG version is the most recent, most cheap and full of extras!

4) Oldies. If you are looking for them, DosBox and CYBERMAGE will leave you asking for more (cybermage is something like the ancestor of system shock which is something like the ancestor of bioshock, and one of the very first games that combined shooters with RPG elements and ability to drive vehicles).

And that are my two cents. I am not a shooter person, but those games were so good, they could convince and entrap into them even a HATER of the genre! Try them, you won't regret it!

I didn't think it was worth the hassle. The levels are slightly varied but not enough to make up for what a pain the game can be. Yeah it looks cool and sounds cool which is okay for a while but it just goes on too long and some of the climbing jumping stuff stops being fun and just becomes frustrating.

Also I just realized I have Shogo on my system and have never played it, I guess I should.

Treasure: The first thing that came to me upon reading your description was the 2 Serious Sam games here on GOG. [...] No regenerating health
Actually Serious Sam 1 has regenerating health on easiest difficulty.

That said, for OP I'd suggest what has been mentioned before
_Serious Sam if you prefer a true 3D game. I'd strongly suggest you play on Medium difficulty. Anything easier and you run the risk of getting bored quickly defeating wave after wave of easy enemies.
_Shadow Warrior classic if you prefer a 2D "3D" game.
Uh hello, the OP just described Deus Ex and no one mentioned it yet?

Vnlr: I was looking for a game along the lines of:

Half-life (1/2)
Unreal (the original, and best one)
Dark Forces
Jedi Knight 1+2 (well ,kind of dark forces II and III, but whatever)
Doom 1+2+3
Duke Nukem 3D
Return to Castle Wolfenstein

[...]
Definitely Blood and Shadow Warrior.

Shogo - Mobile Armor Division is absolutely awesome, if you can get past the ugly graphics and insane difficulty. I'm pretty sure the highest difficulty level is virtually impossible and certain parts of the second highest one are only barely beatable with lots and lots of luck, and exploitation of the AI. Keep in mind that it doesn't really work (well) on Windows 8. (Video review by Avalanche Reviews)

SiN Gold is also a very good candidate, it even features some stealth. (Video review by Gggmanlives)

Clive Barker's Undying is really good. It does play quite a bit like the games you listed, but it also features a lot of original elements. (Video review by Gggmanlives)

BloodRayne might be appropriate if you enjoy games like Max Payne, because it's largely a knock-off of that. The major difference is that it's mostly focused on melee combat, but with lots of disposable guns as secondary weapons. Quite fun, even though it lacks polish somewhat.
Keep in mind that BloodRayne 2 features a very different style of gameplay. It's more of a straight up brawler with some "Prince of Persia"-style platforming.

If you're interested in BloodRayne, you might also want to take a look at Total Overdose. Again, very "Max Payne"-esque gameplay, only really, really silly and over the top. Additionally, it has some rudimentary sandbox elements.

I would recommend XIII and Prey, but those two aren't available anwhere now.

How about FreeDoom?

I almost forgot!

The one that started it all!

YES I know this has been posted a gazzilion times before but it never gets old. XD
By the way, I forgot to mention Hexen! It is made in the Doom-engine and has plenty of melee-combat, depending on which class you play as. It is set in the medieval ages, so theres no guns, but you can shoot spells :)

Strife is another great Doom-engine game. It's a shooter with RPG-elements. I've read that Strife: Veteran Edition is coming to GOG, but I don't know when.

So, apparently Mr. Square went into game development.
the only fps game that I can think of that I've played with a heavy melee focus is Breakdown for Xbox.

Though, it's more of a first person action adventure/beat em up/punch glowing naked Crispin Freeman in the face simulator, with some crappy FPS aspects near the beginning (since there's only a couple guns, there's lockon like in metroid prime and the level design is pretty boring/repetitive until it starts turning into DBZ)
