Like some good lads advised already you should consult with a doctor and give him/her more details regarding "separate beings".
On the other hand it's not necessarily means you have a mental disorder because brain and consciousness (yourself) are indeed two separate being. For example the brain of a drug addict will do anything to get more drugs, but the addict may find his/her actions questionable.
However we don't know how far they stand with each other in your case. You haven't give us any details.

And let's not forget about heart and pancreas (especially).

It's a common symptom of anxiety disorder: http://www.anxietycentre.com/anxiety-symptoms/altered-state-of-consciousness.shtml. As the others have said, you should see a doctor/therapist about it. In the meantime, I'd advise you not to worry too much about it, as that will just make you more stressed and anxious (I speak from personal experience here). Instead, do something you find relaxing and enjoyable to distract yourself.

As much as I know there are good people here on the forums, I'd still suggest you to go see a psychologist if you're feeling a bit off or worried you might be having a psych issue. I don't think you'd be able to find many serious replies on any forum, and even if you were, I don't think a psychological evaluation done behind a keyboard with so little information provided is gonna help much, whereas a professional will have a better understanding of the field, which symptoms to keep an eye on, which info they'll need and an adequate course of treatment in case there is something you gotta work on.

No reason to be afraid of getting your mental health checked, it's just as important as your physical health and both have a connection.

I´m no doctor, but this sounds like a birth defect.
It's very normal, what you need to do is to jump the brain along into the tank, at which point it merges with the coach's brain. At this point you reach the hardest level of the game, I recommend installing the patch if you haven't (latest download includes it), because it's just not fun otherwise.

We are talking about Gwent right?
i don't think that it's a mental disorder at all.

It just sounds a lot like anxiety to me (not diminishing that though.. anxiety can be completely incapacitating). I wouldn't worry too much about the "mental disorder" thing but it might be a good idea to speak to a doc about it because it may be some anxiety you're suffering and they may be able to help.

Of course if it is anxiety, IMO therapy would be far preferable to new drugs in your system so nightcrawler's advice above was the best... that's probably the best type of doc to seek. Not because you're "going crazy" but because they probably offer the best chance of helping you without too many unintended side effects.

Whatever happens I do wish you the very, very best. I can't pray for you because I don't believe in God... but I can hope for you. I hope that all works out well. Good luck.

People are going to tell you to go to a psychologist, but what you probably really need is yoga and zen meditation (I think, only you know for sure)
