There's nothing out there that's quite like Entomorph... that's one of the things I love about it. That said, if you can tell us a little more about what aspects of it appeal to you, we might be able to come up with something sort-of similar.

If you like the setting, you might want to try the other World of Aden RPG, Thunderscape. It's a first-person dungeon crawler, so the gameplay is fairly different, and it's set on the mainland so the races and monsters are a bit more "conventional" for a fantasy setting. I enjoyed it, though, and on the plus side, it has the same awesome music team doing the soundtrack.

Some other games that might appeal (there's not much logic to my suggestions here, just a few games that come to mind):

Divine Divinity: Has some vaguely similar "roving around a top-down worldmap fighting enemies in real-time" gameplay, though with more stat-building and loot-collecting than Entomorph.

Arcanum: If you like the somewhat oddball setting of Entomorph, Arcanum's pseudo-steampunk fantasy might also appeal to you. Can be played in real-time, though I personally find the turn-based mode far more enjoyable.

Heretic Kingdoms: Another top-down ARPG with a slightly unusual fantasy setting. Gameplay is a little more diablo-ish.

Yes nothing I have played since has been quite like it.
I enjoy the oddball setting, and the storyline is quite intriguing. The game has a unique atmosphere. I quite like the top down style, the look of Thunderscape doesn't quite do it for me, though I will probably check it out at some point .
The suggestions you posted look good so I will definitely try one of those out, maybe Arcanum.

Guys, Al-Qadim. It's basically the same game but in another setting.

I love them both to death and I wonder why these developers didn't release more of them.
