I find this game extremely difficult to play with all of the different controls, and I have tried custom control settings but I just can't seem to get comfortable or play easily with them. Any suggestions/recomendations?

Try using the numberpad on the keyboard rather than the usual directional keys as it
provides a fool proof way to shoot diagonally and you get used to it after a while. I found
the other controls to be fine. Perhaps map the weapon change button to somewhere
convenient.

I use joy2key, which works great after disabling the joystick support in the dosbox config
