I'm using the Windows 95 patch. My gamepad is the Saitek P2500 Rumble. The game only recognises face buttons 1 to 4, though there are 6. Doesn't recognise the shoulder buttons or analogue stick buttons. Or the sticks as analogue sticks for that matter. D-pad does work.

Gamepad is fully updated with it's own drivers. Tried it with Carma before and after updating, same situation both ways.

Does the game recognise more buttons than that?

My Logitech gamepad has software that allows me to bind keyboard commands to buttons.
If you're looking for mappingsoftware try JoyToKey.

I don't think it has mapping software, it's ancient, think I got it in 2003. I discovered that the right analog stick does actually work and is analog steering, so that's good. I do have JoyToKey, was just hoping I would avoid using it but it's not a big deal.
You could also try it with Ctrl-F1, though I don't know if the Dosbox mapper will pick up all the buttons.
