I already had vsync and triple buffering turned on in Nvidia control panel, but I still see screen tearing. It's just one tear in the middle of the screen. This is extremely odd because I don't have any tearing in any other game, and this leads me to believe that the culprit is something other than vsync and/or triple buffering, and THAT is just flat-out bizarre. Does anyone have any ideas?

I'm not doing either of those things, and this is the only game that has tearing.

No. I just put up with it.
Just started playing this. For me the Vsync does seem to stop the tearing, but it adds an (imo) unplayable amount of input lag. I hate that a lot more then screen tearing so i'm turning off the Vsync. Too bad my Gsync monitor doesn't seem to be fully working on this game either.

Edit: I turned off GOG's Vsync fix & fps cap using Nvidia Inspector and turned on fast sync in the Nvidia Control panel instead. I also capped the FPS using RTSS. Fast sync by itself still seems to have screen tearing. Fast sync + Gsync seems to have fixed the tearing without introducing any awful input lag. Or if there is some input lag i'm having a hard noticing it compared to Vsync completely off. Gsync monitor still proving to be an amazing purchase. So many older games (and very rarely some newer games) get really bad input lag when using Vsync. Gsync almost always fixes that.

Forcing Vsync to ON and turning turning OFF triple buffering didn't work for me either. I could still see the big tear in the middle of the screen. However, it worked after I added one additional step. I downloaded the widescreen fix at this link: https://thirteenag.github.io/wfp#callofcthulhu

After extracting the Engine folder and placing it in my GOG Cthulhu directory "C:\Program Files (x86)\GOG Galaxy\Games\Call of Cthulhu", I started the game and the screen tear disappeared. I'm now able to run the game at 1920X1080 or 3840X2160.

I still have Vsyn forced to ON in the Nvidia Control panel and triple buffering set to OFF. I don't know if that matters.

I can run the game, too, at 3840x2160, but the diary looks like it does in the link above, unfortunately. So I dropped down to 2560x1440 to get a proper diary read.

I don't have a nVidia card, but an AMD card, and run the game with Vsync off but with Enhanced Sync turned on in the drivers--provides ~90% of the frame-rate of vsync off but with little to no tearing.

Yes, Waltc. Unfortunately, at 3840x2160 my diary does look like the one in your image. I can't read the text properly. I was so excited to not have screen tearing that I didn't notice that detail. I've since tried 2560X1440 and the diary works fine, but I'd rather have 3840x2160. You can see more on the screen at the higher resolution. I might try playing with the FOV.

I'll also try turning vsync off now that I've applied the widescreen fix. I'm also curious if fast sync now works. That's supposed to be the best version of vsync.
