In Baldur's Gate, as far as I know, there is no circumstance that could alter your alignment. (Maybe only using some items.)

I'm not sure about that in Baldur's Gate 2, though. In the last chapter, excluding Throne of Bhaal, you can change your alignment depending on how you solve some quest, but is it the only way (without using some items)?
Besides, I don't know whether you can influence your party members' alignment in some way (without using some items).

And I don't remember in Throne of Bhaal if you could change your alignment, someone else's one or even both.

There are certain NPCs in BG2 who can have alignment changes either through personal quests or dialogue choices, including Anomen, Viconia, Sarevok and maybe others. Beyond that, I don't know of any alignment changes, although it's easy to use Shadowkeeper to change someone's alignment manually if you think the situation warrants it.

The only five (possible) alignment changes (that I can think of) are:

Minsc changes from Neutral/Good to Chaotic/Good during BG1 > SoA transition
Anomen can change to Lawful/Good during his personal quest (SoA)
CHARNAME can change alignment during Hell Trials
Viconia can change to True/Neutral during her romance (ToB)
Sarevok (ToB) if you always respond to him in a 'good' way

This is true. It's also worth noting that in this particular case (where Anomen essentially becomes unhinged), if you have Keldorn in the party also, the two of them will actually fight to the death at some point.

Yes, it is. :)

I think there was something like that in Baldur's Gate 2, too. I'm just not sure if it was a helm.
