Today I downloaded and install the game, when I try to install the patch it give me a message that "This update is incompatible with the installed version".

The install file version is 2.0.0.21 and the patch version is 1.0.3.18. Do I even need to install the patch or is it for older versions of the game? and if so, could I played it anyway or the patch is vital to the game?

Looks like the GOG installer installs version 1.03.17.1781 despite the title screen showing 1.00.16.8760 so you probably don't need the patch to 1.03.18 to be up-to-date. It doesn't work on my system either. It's just a subversion of 1.03 maybe due to a later release date (e.g. patches for GOG versions and Steam version sometimes have different subversion numbers, although addressing the exact same patch content). My guess is, that 1.03.17.1781 is up-to-date and 1.03.18 is the version number of the patch for GOG's standalone version of the game, and only provided for people who have already installed an older version of the game.

(GOG's installer version numbers never correspond with game versions, so 2.0.0.21 isn't the game's version but only the installer's version)

The other languages are missing and of course I can't install patch 1.0.3.18

Patch 1.0.3.18 changelogg
- Added new text locales for Czech, Hungarian, Polish and Russian languages.

So, someone made a BIG mistake.
v2.0.0.21 comes completly WITHOUT these translations, what now?!

The only way to get proper translation is 1.0.0.13+1.0.3.18

GOG please FIX that and made a patch for v2.0.0.21

Contact customer support directly rather than posting on the forum, it is too big for them to check every thread on a whim..
I've found a way to workaround this problem.

Go to HKEY_LOCAL_MACHINE\SOFTWARE\Wow6432Node\GOG.com\GOGALANWAKEAMERICANNIGHTMARE\
and change BUILD to 13 then install 1.0.3.18 patch.

It's pretty safe procedure since installer only add files instead patching anyhing.
