Pathologic 3 is a psychological horror game where you have only 12 days to save a town from a mysterious plague.
You are Daniil Dankovsky: physician, researcher, prodigy. Your research into the nature of death has brought you to this remote town deep in the eastern steppe, looking for a man said to be immortal.
But the outbreak of a deadly contagion sweeps through the town. The immortal man is dead. You failed to stop it.
Now you must return to the beginning. Fix your mistakes. Save the town.
Every minute counts. Save them all, discover the secrets of immortality... don't go mad in the process.
Bring your medical training to bear. Observation and knowledge are your tools. Work with patiens face-to-face.
Examine the sick and make diagnoses. Each patient is a unique story — and a medical mystery. They may lie, omit the truth, or hide their motives. You must separate fact from fiction, and identify the real symptoms. Step by step, you'll get closer to developing a vaccine that could change everything.
Fight for every new day. Ensure the town's survival by imposing your own rules: enforce quarantines, confiscate precious medicine, vaccinate the population, suppress riots, deploy patrols, and declare curfews.
You will make mistakes, face their consequences, and unlock new decrees. Your actions will shape the town—even if the people grow to hate you for it. Power is in your hands. It's up to you whether the streets will see gunfire or fireworks.
Made a mistake? Go back, and fix it.
Time is your tool for navigating a narrative web filled with moral dilemmas and branching paths. Rewind if you think you could have done better. Keep your eye on your ultimate goal—even if the road there is paved with sacrifices.
How far will you go to find the truth? Your mental state directly affects your actions, and even how you interpret events. Falling into apathy or wandering into mania may lead you to new insights... or to your death.

Filled with so new ideas and ice-pick lodge's signature dreamlike storytelling. A game series and studio worth supporting. Absoutely the most distinct game in the series too. so even if you bounced off any of the others, you might still love this one.
Feels like a privilege to get a chance to return to the town after so many years. Love this world like it's a part of myself. And give it a chance, you might just find it become a part of you aswell.
Highly recommend :)
UPDATED after completing the game:
Summary of Review:
Having finished the game, I'm still in love with the world and narrative of Pathologic, and have been since my very first steps in P1HD. P3 also unveiled a lot of mysteries that had lingered in both P1 and P2. However, I would not say that P3 is the best entry point into the series. I believe that honor remains with P2, or even P1 were it not for the slow movement speed and outdated graphics.
P3 really reminds me of video game philosophy from the late 90's and early 00's. What that means by today's standards, is that the game is difficult in its mechanics, and has very little in the way of clearly defined goals (in spite of way markers, and the thought journal). But the real difficulty is in the player connecting all the dots themselves. Overall, I really hope that more Pathologic games are made, and am more than ready for dozens more go-arounds in this game, future entries, and DLC.
Review:
I really enjoyed both Pathologic 1 and 2, and 3 shaped up to be just as interesting, if not moreso. However, even as a seasoned player of the series, I found this go around to be far more of a confusing struggle. Not that it was difficult gameplay wise, but in its puzzle-form narrative requiring one piece it together out of order. Though, that's not necessarily bad, and this game just took quite a bit more adjustment time than the first two.
As I said, the narrative is quite difficult to follow due to the time travel hijinks, and it was the repetitive nature of re-visiting completed goals/days that proved the most bothersome thing to me. I may have been unique in this, in that I had to return to a certain day (Day 2) about eight times before I figured out why I couldn't get it right. More on that later.
The game mechanics are not properly explained IMO (not that I expect or desire hand-holding). These issues are persistent for the first half of the game until you figure them out yourself. For example, early on I had no clue how to make any medicine, or really use any of the apparatus in the Stillwater (besides the microscope which was learned in the prologue). Subsequent daily goals become the training ground where I was forced to figure out how to use them or fail. It's honestly refreshing to find a game with a lack of hand-holding, though I wish these mechanics were explained in the tutorial prologue.
My Primary Gripe:
The biggest headache I had, which may/may not have been intended by the developers, is the fact that daily progress does not save unless you complete the day. That means, you need to wait until 2AM for that day's progress to save. If, for example, you jump back in time and alter a day's events, then time-jump to another day before 2AM, you'll have lost that day's progress. This issue caused me massive headaches, with annoying repetition, and nearly dried up all of my playthrough's sources of amalgam scattered in mirrors across all days before I figured it out. Day 2, in particular, proved to be difficult to get the results to stick into future days. Exacerbating this problem is the fact that the game tells you something along the lines of "my choice echoes into the future," which indicates that you should then move forward at any of the many given clocks scattered in important areas. DON'T DO IT! Sleep till 2AM, first. For this reason, the only place I used time travel was at the Stillwater.
I would give P3 a 4.5/5 but GOG does not allow partial ratings, so I give it a 5/5 because P3 is an extremely bold and unorthodox game - even among Ice Pick Lodge's own lineup - and subsequently wish to encourage them to make more.
