There's an unofficial patch, that adds higher resolutions and correct errors that the developers never gotten around to fix.
You can grab it here (browse to the Download section).

BE SURE TO RENAME THE PATCHED PK3, OTHERWISE IT WILL OVERWRITE THE ORIGINAL

As a little bonus. If you play Quake 2, either on Gog, CD or on Steam, on the same page with the patch there's also a source port for it. It's called KMQuake2, but IIRC it was called Knightmare Quake 2 back in the day. You could either use KMQuake2 when you're about to play Q2 on a modern setup (mission packs works too!), or you can use the unofficial patch for Quake 2. Yes, there's also an unofficial patch for Quake 2 on that site. This adds higher resolutions and fixing some errors, but doesn't add eye candy like KMQuake 2 does.

Have fun!

I can also highly recommend that RTCW patch mentioned!

Seeing the game in widescreen and everything is quite cool.

Will check out Quake II patch as well, though I have the retail CD and it works fine on my modern PC :)
The patch as-is overwrites Main/sp_pak4.pk3.
Find that file in the patch zip and rename it to sp_pak5.pk3.

How do get this to work? I installed and then changed resolution to 1920x1080, but then it screwed up by zooming in way too much. I assume it has something to do with the game using 3Dfx mode? How do I disable 3Dfx mode?

Edit: Figured it out, disable 3dfx by finding "seta r_glIgnoreWicked3D "0" in the config and change the 0 to a 1

Does RTCW set the fov by itself or something? I don't see any fov options in the config file (I'm using the unofficial patch too btw).

If you want some enhanced textures, effects and other fixes and features besides widescreen resolution, do not miss this cool mod...definitely worth trying.

Here are the differences in their contents.
If you don't rename the patch one, it will completely replace the original one, not merge with it.

Ah, I had no clue. Thanks!
So we should have both, but rename the patched?

Precisely.
Tried downloading and installing the rtcw unoffical patch on windows 8.1 but the game just crashes on start up, telling me the exe has stopped working. even using compatibility mode does nothing. Strange thing is I can get Quake 3 to run no problem and I believe this game uses the same engine. Any help would be appreciated.

I had the same problem on Windows 8.1 (WolfSP.exe stopped working) but the suggested unofficial patch worked for me: now the game starts.
I can play in my notebook screen native resolution (1366x768)
There's also ioRtCW, should you still have issues getting the game to run in Windows 7/8/8.1/10.

Currently at version 1.42d.
