I made it to Act 1 "Hard Landing" of the game, but I'm noticing some problems.

1) When I encountered Iorveth in the forest, I was presented with some choices. Before I could even press a button, it simply picked the one that I had highlighted about whispering to Triss that Iorveth needed to be taken alive without ever receiving any actual input from me. As far as I can tell, there hasn't been anything in the game that suggested some conversations may be timed.

2) When I started a new game, I imported my save from the first game. However, whenever I equip Geralt with the Raven's Armor, it ends up being switched out for the Blue Stripes jacket any time I started a new section of the game (like going through each section of the Prologue and starting Act 1), which means I have to re-equip the Raven's Armor every time I regain control of Geralt.

3) Ever since I got away from the dragon in the prologue, I have not been able to use the Meditation feature. I tried to meditate after jumping off the ship in Act 1, but it didn't do anything and I know that if it couldn't be used, the game would say so but it doesn't even do that.

There have been several timed conversations and timed actions in the game from the beginning. This has always been one of them, not something that was changed in a recent patch. Did you notice an hourglass icon next to the speech choices? That indicates that your decision is timed.

The Prologue has a bad habit of re-equipping your original equipment (Blue Stripes armor, original swords) after each segment. This is a bug of long standing and was not introduced in a recent patch.

Meditation being broken is a new one on me. I have always been able to meditate or gotten a "you can't do that now" message. I'm also running Patch 2.1, recently played through the Prologue and "A Hard Landing", and didn't notice anything of the kind.

If you actually aren't getting, as opposed to not noticing, the hourglass icon or the message telling you that you aren't allowed to mediate, then you may have an issue with your hardware, because others aren't reporting similar problems. What graphics hardware and what driver version are you using?
You can't stop and meditate during the introductory section to Chapter 1, which lasts until you arrive in Flotsam itself. It's not a bug, but is intentional. However, if you still can't meditate once you get into Flotsam, it may be a problem.

There aren't many decisions in the game that need to be made quickly, but the one at the first encounter with Iorveth is, as you saw, one of them. Was the hourglass there?

The constant swap between the two armours during the Prologue is apparently intentional. Yes, I agree that it's also annoying, but it's over now.

All of these have been present in all versions of the game so far.

Exactly, is because Geralt is telling a story. Is not part of the "real time". Annoting, but nice ;P

Annoying but with benefits (like the consumables also resetting, and giving you the chance to recover if you waste a valuable rune on a poor sword).

I can understand why they did it, and it's not a big deal. It's not as though they bring him in almost naked every time and you have to re-equip :)
