Is there a good description on how combat is supposed to work? I think I am doing it correctly - but I am not managing to hit anything. And then the lame popup happens explaining something about a flaming sword. and waiting but all that does I stand around and get hit. I had always heard that the combat was broken, I guess they were right.

I'm thinking you might be clicking to frequently. I'm using the OTS view center the cross hairs on the guy I want to attack and then click and then click again every time the sword goes flaming making sure to keep the cross hairs on the target. I'm new to this game and it took me a bit to not just click like crazy with the mouse. Just my thoughts.
Like Gator_man said, only click the mouse after your first attack when the sword is flaming. Clicking any sooner cancels the previous attack.

I also recommend using the OTS mode when not in a fight or when facing one enemy but then using the low isometric mode when facing multiple. I find it easier to move around and not get surrounded in the low iso view. Use F2 and F3 to switch between the two.

In case you didn't notice, the flaming sword refers to the mouse icon. Not the character's sword in the game.

Another thing to remember is that you have to actually click ON the enemy. You can't just click to swing your sword like in other games. You must have the mouse cursor on the enemy. Then, click once, and wait for Geralt to attack. After he's swung his sword a couple of times, you should see the mouse pointer become a flaming sword (provided you still have the mouse cursor over the enemy). Once you see that, click again to initiate the second attack. If you click at any time BEFORE the flaming sword shows up, the attack will be cancelled. It's all about timing your clicks.

Once you get the hang of it, it's easy to do. Later on you will learn new attacks in the "sequence", so your second attack will be more badass than the first one, and so on.

I think I am starting to get the hang of it - I was having trouble seeing and then timing the flaming sword and that was getting frustrating.

On easy and normal it refers to mouse pointer changing shape, but on hard it refers to the actual sword.

Are you saying that on Hard mode the actual sword indicates when to complete a combo?

I never notice any changes using hard (hi-texturen mod), thought you just got use to the animations for the timings!!

I have only played on Hard, so I don't know about the others, but on Hard the indicator for when you should initiate the next attack comes from looking at Geralt's actual sword. When his attack is finished, he swings it very quickly in front of himself and the blade itself has some fire-streaks to it as he swings. This is accompanied by a sword-swoosh sound effect. Apparently you can turn the flame part off in the options and just go with the audio cue but I didn't bother.

Apparently the Hard setting also narrows the window in which you can click for the next attack, so you need to be more precise with timing than on the easier settings.

Unfortunately that stops working once you learn more combat moves, because some of htem take longer than others. But as long as you click as soon as you see that sword-swoosh, you should be OK.
It's a bug ... i've got it right for the first combat, i tried during one hour a lot of thing when clicking... and it's a fucking bug, i can't play ... can't do any damage.

EDIT : Not a bug, it's me ... i was thinking it's like an hack n slash, i click non stop xD ... to bad next i wil shut my fuck up lol.

Sorry, now i'm stuck can't find the button to meditate ... i feel like a noob

If you hadn't already figured it out on your own, you can only meditate at campfires, or certain other locations when talking to an NPC.
The simplest combat in all three games and no one can work it out. That is pretty funny. :P
