I've just purchased the game, my first observation was there are no speech during in-game dialogues (when you talk to other NPCs), only subtitles. Why? The voice acting was top notch in this game when I played the cd version some years ago.
This question / problem has been solved by Firekimage
Same problem here, and I'm using Windows 7.

OK, we've got a workaround for this problem, but it's not perfect yet. If you wish to try it out, please download the archive found here and unpack it into Sanitarium's folder (thus replacing the current sntrm.exe file with the one you downloaded). The "not perfect" part means that the game works great only until you change chapters.
This means there will be no problems when:
1) You launch the game and start a new game,
2) You launch the game, load chapter A and play it.
But all dialogue lines will be skipped if you load a different chapter:
1) When you play chapter A and load chapter B from a saved game,
2) When you finish chapter A and progress to chapter B.
To get around this, you need to save the game at the start of each new chapter, close it (Alt-F4 to make it quick), restart it and load the saved game. We'll see if we can come up with a version with this problem fixed. Until then please use this temporary fix if you wish to enjoy the "talkie" dialogue lines.

Believe me it gives us much better gaming experience with voice... those creepy children voices in chapter 2.. Brrr..

Now there's a new problem, the sntrm file now crashes quite often.

The game is keeping crashing ramdomly.
The patch did wonders for the dialogs but not for stabiliy.
Crashes are random and never twice at the same place.
Save often.
I'm seeing tons and tons of random crashes as well with the updated exe.

No crashes so far (only about 1/2 hour play though) and it actually kept voices going between chapters! (XP)
For those who experience frequent crashes: try running the game in windowed mode, something like this:
in Start menu -> Run... -> "C:\Program Files\GOG.com\Sanitarium\sntrm.exe" -w
Notice the -w switch at the end of the line. It might help.
And last but not least follow the golden rule: save often (in different slots).
I'm soo glad thats sorted..... it really does harm the atmosphere without the voices :)
i have to say... this worked :) and restarting the app on new chapters isnt that big a deal, not like your changing chapters every few minutes
Running the game windowed worked like a charm, no crashes since.
