Hi,
I don´t understand all the fuss about running this game in proper fullscreen mode (e.g. no borders). If you have nVidia, enter Control Panel, and add custom resolution 512x384. Then, start loader.exe, uncheck Frame mode, and .. here you are.
There is no need for any patches, which make game unstable and WITHOUT DOF (Depth of Field), no need to tinker with ini files and write permisions.

But will this work with all monitors? Setting monitors to work at odd resolutions and refresh rates can damage them. I've checked the nvidia Control Panel and it does give a warning message.
"Warning: The software utility you are about to enable ("utility") may potentially cause damage to your display monitor and void warranties, if any, in connection with your display monitor.
This utility may run your display monitor out of the manufacturer's design specifications, including, but not limited to: higher system voltages, above normal temperatures, higher frequencies, disruptions to sync, and changes to bios that may corrupt the bios.

Well, my guess is that this applies to some extreme high resolutions on some extreme old CRTs - when you use something as minimalistic as 512x384, monitor doesnt have any reason to use higher voltages or frequencies.
I checked out google for "nvidia custom resolutions + damage" and there is NOBODY who screwed up his monitor this way. nVidia is just covering its ass, don´t let em scare you. From my point of view - it is totally safe, modern monitors (<10 years) have safety circuits which would never allow them to run on some dangerous frequencies or voltages.

Well, there's no such option in the Nvidia control panel on my Alienware m11x. I cannot get the thing to upscale to the full screen, and it's really annoying in the tiny little window.
I've tried the custom patch, I've tried changing the compatability settings, nothing works.
That approach does not work for all monitors.

The option is located by first selecting the Display - Change resolution option from the sidebar task list, then in the main window choose the Add Resolutions button. A pop-up window appears and you can the click the Create Custom Resolution button to create a new resolution with custom settings. You will then get a warning message before you can access the tool.

Be specific - for which monitors doesn´t it work?

There is no "Add Resolution" button on mine.

What version of the nvidia drivers are you using and do you have nvidia system tools installed? If so, what version of system tools?

I bought a BenQ 20" monitor a year ago... No resolutions below 640x480 are possible.
The graphics card can show the resolution, but the monitor can't. It pops up a little text that says "Out of range !".
