i had the same issue with grey or no game screen and error ddraw error.

I use Windows 10.
After i run the game with the compatible mode, 640x480, as Admin and with 16bit color it worked perfectly fine.

I included a picture of the settings.

These settings worked for me as well, thank you!
I tried the compatibility mode settings kindly recommended for Win 10 but no dice. Still get direct draw error :-(

If you haven't already I would recommend trying to install the greyface patch as posted above.

Also if you downloaded it through GoG Galaxy you could try the DRM free download instead or vice versa I remember Kotor not working with one but worked with the other one. I know that's a different games but the same concept applies lol

ok i got it to work by going into the properties-reduced color mode to 16-bit-under compatibility mode run as windows 98/windows Me, i do not know if this will work for everyone but yeah
Yesterday I installed the game (via GoG Galaxy) and played it a few hours without any issues. Important to say - I also quit it and later evening launched it again (via GoG Galaxy) and once again no issue at all.

Today after launching GoG Galaxy the first thing I see is that MM6 on my PC was updated (I can also see it in Downloads) I run the game and immediately have this error. Running it directly via .exe does not help. Re-installing the game did not help.

Setting 8bit color mode of the .exe file helps and then I can run the game fine. But since that moment GoG Galaxy kind of "loses link" to the game, does not track time played etc. Once I verify the installation via GoG Galaxy, the error is back. So it seems to me like GoG Galaxy enforces some setting that does not work.
Well I have mine on windows 10, compatibility 98, admin, 640x480, 16 bit, the game runs, at least i can hear it running, but there is no window to see anything, if i change the window to maximized it can see but its distorted.

I got the same error on windows 10 with mm6 installed from GOG. In the end, I got it running like this:

1) In GOG, download offline backup game installer and install it.
At this point I got the ddraw error
2) Install grayface patch mentioned in this thread
No compatibility settings needed for me. Now when clicking the exe, the app starts but is minified in the task bar and doesn't seem to open.
3) hover on mm6 icon in the task bar, a preview of the window appears above it. Hover on the preview, right click on it. Choose "maximize"

This way the game runs ok. Then press F4 for fullsreen. On next launches, it always starts fullscreen without the need for maximize -step. Ofcourse the Grayface patch brings new stuff to the game, but hey, at least it runs, and the changes are generally seen as good :).

The person above mentioned that the game was distorted. Mine was too when compatibility color mode was active, but removing the compatibility check boxes helped it render properly.
