Tomb Raider (3 I think)
The "Most accidents happen at home" Unachievement
Kill Lara during training by making her do a swan dive off the balcony at Croft Manor.
It's actually really easy to do ONCE you know how to do it, that's why I'm not giving away any of that stuff. :P
EDIT: I just checked, it is #3.
Don't even ask me how I figured out you could do it because NO, I DID NOT read about it somewhere. XD
