I am not even surprised it's so great considering it is a mod for Circle of the goddamn Moon (already an incredible game and one of the first I played)

rampancy: +1. I'm usually a purist when it comes to games like these, but I found Brutal Doom to be the best all-around mod I've seen for Doom. I've actually wanted to know if there's a similar style mod for Quake, which offers gameplay improvements, graphical tweaks and extra features.
Emob78: I'm glad we agree. Apparently, we're in a very small minority. The hipster 'gotta play it old school' puritanism with Doom is kinda silly to me. Brutal Doom takes a great game and puts a rocket launcher and jetpack on it. It's just pure adrenaline fun. Nothing wrong at all with vanilla Doom, but for a ramped up experience of fuego gaming, it's quite an achievement. Overall I'd say Brutal Doom relies more on visual flair and gimmicks rather than actual gameplay

Well, besides the gory visual aspect (which lots of us really appreciate), it turns DOOM into a new different game, with the base of our beloved shooter but with totally different mechanics besides the graphical additions (which even if you don't like them, are pretty cool, are a curious addon to a game which looks essentially like it did in the 90's and don't really lag that much). You can play Doom for a while and then, by just loading a file, you get a new game which shares the theme and levels, yet plays totally different. Unload the file, and you get the original experience. A win/win situation if you ask me.
I can see how somebody won't appreciate the excesive gore or won't like the modern features applied to such a classic. But wether you like it or not, it is a great original mod, whose gameplay is superb (I can't think about any FPS packing more adrenaline that BD in highest difficulty).
Also, you can disable the tasty-cles via options if you don't like them (which is a pretty sensible and common thing tough).

While I know that the blood effects are of high quality and took some effort to make, the "WOW" factor wears off after a while and you're left with bunch of useless crap the game has to render. Really, graphical effects should be NOT be a priority for game makers. Sure, it might look impressive at the moment but people will eventually move on from whatever high-quality skin detail you worked oh-so hard on. and correction: They do lag, A LOT.

As long as said file contains Project MSX and not Brutal Doom, THEN its a win/win situation if you ask me.
