Those were just my recommendations. Inverting confuses me in shooters. I guess my brain just doesn't process movement like that. If you want Duke to look down when you move the mouse up and up when you move the mouse down then you'd want mouse aiming flip off. I believe that's the default anyway and I should update my original post. Thanks for bringing it up. Is that what you meant? Because that's what inverted means to me, up is down and down is up. Otherwise if turning mouse aiming flip to off doesn't do what you want you could try what CoreySteel is asking you to do by changing the mouse settings in Controller Setup.

I found a better way to set it up. If you do what you stated to do, the game uses the digital controls. It does work, but aiming up/down is kind of slow (at least for me it was). What i did was i changed "Mouse Y Axis" to "Analog Moving". Then in game just turn on "Mouse Aim" and it should be set up just like a modern day FPS. The up/down is a lot smoother and faster in the analog mode than it is with the digital setup. Although aiming up and down sort of distorts the graphics due to a lack of perspective correction. But it will do that with the digital setup as well.

Hope that helps you little bit. That's how i play it at least. It took me a little while to get a good configuration. (Also even though it is changed to "Analog Moving", it won't move you forward/back with the mouse if you have "Mouse Aiming" turned on)

If you are referring to the Mouse Aiming Key "U". What it means is it unlocks the mouse to free aim. If it is off you just move forwards, backwards, turn left and right. All BUILD games allowed this, examples like Shadow Warrior, BLOOD, Redneck Rampage and Duke3D.
This has been mentioned in many forums about "BUILD" games. But there is a program called bMOUSE. Current version is v0.6. here is the website http://swisscm.duke4.net/

There is a included help file that will explain to you how to use it and run it every time you enter the game.

I spent 5 minutes fighting controls and got the EDuke32 engine. Real easy, auto-detects your GOG install and uses modern FPS-style WASD/Mouse-aim controls. Plus fixes tons of bugs from original game, doesn't emulate but runs native in windows, list goes on. Best option.
Mine (eDuke32):

W,A,S,D - Move/Strafe
Space - Jump
Ctrl - Crouch
C - Holoduke
F - Jetpack
V - Night Vision
E - Medkit
Shift - Steroids
Q - Quick Kick

Mouse1 - Fire
Mouse2 - Open
Wheel Up - Previous Weapon
Wheel Down - Next Weapon
