Sure, some enemies may look kinda freakish and that screaming Phantasm enemy that looks like Death Jr. sometimes scares me if I open a door and it stands behind it.

I guess I picked Blood up too late in time to be affected by it in a scary fashion.
When I first played it it was several years after its release and there had been some games in between.

As you mentioned, only the Phantasm ever really scared me. Or rather, it shocked the hell out of me the first time I encountered it.
But otherwise, there's just no time to get scared since you start spamming rockets and dynamite as soon as you encounter a huge scary monster. Altough those situations get very tense, they're always more of an adrenaline rush than a scare, atleast for me.

But yeah, if I'd have played this as a kid, I'm sure I would have freaked and gotten nightmares from things like the meat merry-go-round in the carnival level. Especially since I probably wouldn't have understood all the humorous elements in the game.

To me, Blood is pure fun, in a very well-made horror setting.

To be honest when I first played it (in 2000, I think) it did scare the shit out of me. Nothing in particular, rather the overall atmosphere. Let's say it was more disturbing than scary.
Actually, I found the level The Haunting to be fairly scary.
Having just finished a playthrough, I jumped several times - usually the aforementioned phantasm, but a couple of times when I dropped into a corridor filled with the shark men...
Scared me back in the 90s not so much now. It scared me more then hexen did back then and hexen had me white knuckling through some parts.

Only thing about either Blood game that unnerved me were the crawling severed hands. The way they skitter rapidly directly toward you -- ugh!

Other than that, not to terribly much.

well, not so much "Scary" as intimidating, when I was younger. I ran into this game back in my freshmen years in high school though less than legal means, and at that time I was seeking out pretty much anything violent and horror related. although earlier, I was playing this and one of those reaper guys caught me off guard and made me jump a little....
Seems those Phantasms is the scariest enemy for most of us. :P
They definitely are. That's why I added their screams to the Deathshroud in my Blood 2 mod and also reconfigured their combat tactics to make them appear as close to u as possible before revealing themselves as they scream. Freaky stuff.
I wouldn't call the game scary, I think chilling or disturbing would be closer to the mark. For me, I that hotel level with all the secret passages was the "scariest one". Loved the feeling of that level so much :)
Not really, but the phantom enemies made me jump the first time I played the game though. Mostly their weird scream and their charging movements that got to me.
I found "The Haunting" and "Crystal Lake" levels quite creepy and unnerving.
