Ok, so we kill Irenicus in Suldanessellar. Then we really kill him in the Nine Hells. When you die in the Nine Hells, you really die. But then as an end to BG2, there is a cutscene where Irenicus is being attacked by demons on this fiery realm. How is he still alive? Is this a hint to some kind of infinite death loop, a new horrorable plane every time he is killed? What is going on in this cutscene?

It's a Bhaal thing. Irenicus didn't die in Suldanesselar, not really, since he still had part of Bhaal's (your) essence, same as you and Imoen. There is no permanent death for any who do until it is utterly removed. This is explained at the conclusion of Throne of Bhaal.

Hickory: It's a Bhaal thing. Irenicus didn't die in Suldanesselar, not really, since he still had part of Bhaal's (your) essence, same as you and Imoen. There is no permanent death for any who do until it is utterly removed. This is explained at the conclusion of Throne of Bhaal.
I'm with the idea that Irenicus went to the Plane of the hell accord to his alignment. The "hell" where you has the final fight with Irenicus is Bhaal's realm (your stronghold in the expansion) there you take away all your soul from Irenicus.
