My main dual-classed over to mage, and I decided to restart the game once I did this (ok, FINE, I leeched some free levels off of Throne of Bhaal and THEN started over). I cast summon familiar and seem to remember getting a faerie dragon.

Flash forward and I am a good ways into the game. I suddenly remember I'm supposed to have a familiar, but can't find it anywhere. The spell "find familiar" tells me I already have one when I try to cast it. I have looked everywhere in my inventory and can't find it. The only thing I can find that is out of place is a genie lamp that I'm pretty sure I shouldn't have (I have the sword of chaos, before you ask). I also have no constitution loss.

Any ideas on what happened/how I can fix it? I'm okay with cheating to fix the problem if it comes down to it, but I'd like to avoid it if possible.

The game engine checks your alignment when you put your familiar into or out-of your inventory... So removing it from your bag while you are suffering from any kind of alignment change will cause it to disappear. It is also conceivable that you lost control of it while transiting areas but that’s much harder to fix because the game doesn’t save that info in your save game.
The safe way to fix this problem is to add a familiar into your inventory with sk (Shadow keeper).

Just match the type of familiar to your alignment and that should fix it.

Eg. If I was Neutral Evil then I’d need a Dust Mephit

The hit points and armour don’t matter because the game will correct for this the first time you take the familiar out of your pack but you must be the same alignment as when you first summoned your original familiar. If you want to be say Lawful Good instead of Lawful Neutral then you have to kill your Ferret before changing or the game will bugout.

Lawful Neutral: Ferret

Chaotic Evil: Quasit
