I have some trouble setting up the high res mod I have resigned to using 800-600(that one works but is big) my current screen res outside of the game is 1280-1024 but writing that makes the screen weird (it is too narrow to fill the screen it is high enough though)
What setting is best for a 19 inch screen? Is there any rule of thumb I should follow?

Can I see a acreen shot? After I installed the patch I had to tweek it a little but it still looks a little odd because the menu bars don't go all the way accross the screen.

and thats what I mean the screen and the game doesn't fit eachother unless its 800-600 and that sorts of defeats the point

Well, I don't know about that but ultimately it's preference. I do know with the patch the maps are A LOT easier to follow and I personally think the added detail of the players and environment make it worth the menu bars.
Hmm will fiddle some more then
it crashes at 1024-800, 1024-1024 and 1280-1280

Hit Print Screen while in game. Then close/minimize the game and paste the image into an image viewer or a program like Paint. You can also just get a screenshot program like FRAPS.

Those are some weird resolutions you're trying. You need to use resolutions supported by your graphics card, first of all. Double check the native resolution of your monitor, and use an aspect ratio that matches it. There's no way 1280x1280 is going to look right. Try 1280x960, or if you have a widescreen monitor, 1280x720 or 1280x800.

You also need to make sure you have your card set to scale while preserving the aspect ratio, otherwise it will just stretch it to fit the screen which can distort things.

I use 1280-1024 as native that looks fine.
A tad off topic but for games like this I actually keep the resolution a little less for sake of size. I learned this when I picked up a high resolution patch for D2 which made the game look sharper but also made everything SO GODDAMN SMALL I had to get right up to the monitor to see what I was doing! :P I did get Arcanum looking good on high high res but my eyesight isn't so good that I can notice a bottle or a note laying in the grass at that distance.

I have tried everything larger than 800-600 and they are all of some cant run and the rest does not fill the screen I think I'll stick with 800-600 then
