I had the problem with flickering and mouse trails. Running two AMD 7770 cards in Crossfire mode. Disabled crossfire for Arcanum and the problem disappeared. Running perfectly now.

Try renaming ddraw.dll in the root directory...it's an old version of the file and renaming it allows the game to use the up to date version of ddraw.dll in your system directory. Made a huge improvement for me.

2021 now, but this solved my problem, disabling hardware acceleration would just cause the game to load into a black screen and then freeze on whatever frame it was rendering as soon as you alt-tabbed out and back into it

I changed the resolution from the High res Config.ini to the original one 800x600 , changed to Windowed Mode and set render to Software mode. Of course setting the compatibility mode to Windows XP and disabling full screen optimizations

Also used the Major Geeks method

Just thought I'd add in what worked for me. I have an AMD RX 6800 XT. Everything worked fine in the game but the movies. In the Radeon software, I turned on Radeon Enhanced Sync. No problems after that.

Renaming the ddraw.dll to 000.dll IMMEDIATELY solved all my problems!

I was getting desperate after having invested dozens of hours, because the problem was gradually getting worse to the point that even standing still I encountered major video glitches.

Now it is all gone!
