i just installed this on my mac today, and to test if it was working ran it, and everything worked fine, cursor, menu and everything, all smooth. then i downloaded the unofficial patch, the high res town map patch and tried it again. all good.
finally i tried the widescreen patch...what a mistake! it installed fine, but it mangled the main menu screen so there's like toolbars on the top and bottom of the screen. and guess what? it's a higher resolution, but not widecreen anyway. not only that, but now my cursor disappears anytime i try to move it on the menu screen.i can hit "S' for single player, and navigate the mennu like that, but i can't get past character creation without a cursor.
so i uninstalled the patch, and reloaded it up. the menu looked fine (no toolbars) but now the cursor is still stuck disappearing! wtf is that! it was clearly a problem that the widescreen patch created, but it's not going away when i uninstall it. wtf! help, someone. please.
btw, i'm using wine, and have tried both with and without -no3d

i figure it out on my own.

I've never tried running on Wine, so I can't help you. But I do know that when you use the high-res mod, the main menu will not fill the whole screen, because it's a 2D graphic that can't be scaled. So that much is normal, at least.

As for the mouse behavior, I would think that uninstalling and reinstalling the game would fix it.

by "figured it out", i mean, when i was playing it, i'd have to open the game over and over until finally the mouse DIDN'T disappear on the menu. basically i'd open the game over and over, move the cursor, it'd disappear, then i'd hit E, rinse and repeat. about 1 of every 7 boots got me a usable cursor. madness, if you ask me.
What did you expect? Your original post contains hardly any useful info and is pretty damn whiney to be blunt. Which version of Wine? Which videocard and drivers? What settings did you try in Configure Wine and Winetricks? You want a random guess? "Automatically capture mouse in full screen windows."

Honestly if you want help provide as much information as you can and don't get all emotional. "thanks for nothing, community." Get real, no one here is under any obligation to help you in any way.
