- key moves in reverse though. Would this maybe be a dosbox issue? I've never really used my plus key in a dosbox game. Yes, my plus key works outside of dosbox and this game.
I can't think of what would be going on other than a keyboard compatibility issue but even
that seems ubsurd. Any help would be appreciated.

Try pressing numlock and using the keys on the far right see if that works. If not you can remap keys in dosbox. If you press Ctrl+F1 it will allow you to remap the buttons maybe that will get it working?
