I am stuck at the lodge trying to escape being killed when he walks out of the shower. I have gone over this part 10+ times. Each time I put everything back, close the doors and sit in the closet before taking his shirt/pants to activate the sequence where he answers the phone. Every time no matter what I end up dead. I even resorted to the in game system to make sure all the check boxes were checked off before hiding and everything seemed in order.
I am sure it is probably something really obvious I am missing, but I am concerned it could be bugging out and not letting me proceed.

Maybe you forgot one of the following:
Close the balcony door
Put the wallet on the table
Close all drawers
Move the couch pillows to their original position
Put the clothes in the closet
Also if I remember right the scene is triggered when you take something from a kitchen drawer...

One thing I noticed (though it may just be my skewed perspective on it after nutting my monitor in frustration) is that he will get you if you do anything that isn't on the hints list. Every time I picked up the bible and/or the code sheet from under the cushion on the couch I got killed. When I left them both I didn't.
Whether that's definitely the case or not I don't know, but if you are still stuck it might be worth giving it a try.

Well, I'll give you the way I do it myself (work each time) but of course, spoilers involved so those who are not there... don't read.
First, in this part of the game, there is only one thing you really need to do: take the briefcase and a hint on opening it. Everything else (aside a little something) is secondary and you can come back later to check what you may have missed.
So:
Enter the lodge and immediately close the balcony door behind you. Take a look at the briefcase, Tex notices that it is handcuffed so you have to find the key. Turn around a pick up the wallet, take what's inside and put back the wallet on the table. Open the closet doors and pick up the clothes. Search them, you find a key but sadly not the right one. Put the clothes back in place and DON'T shut the doors. It will save you some time later. Go to the kitchen and open the top right drawer (the closest to the door you came in -- at this point it's a good idea to save the game -- Now examine the little something in the drawer, position yourself so you can conveniently reach the door without being forced to turn around then pick up the vial. At this point the phone should ring. Close the drawer (required) go through the door then into the closet, turn around and close the closet doors. You don't need to worry about the kitchen door, it closes itself.
It's safer if you don't move or pickup anything else in the lodge aside the items mentioned above before Slade leaves the bathroom. The missing vial won't be noticed, other missing items will generally be bad for Tex.
I suggest using the keyboard to move through this sequence, I find it quicker than the mouse.

I wasn't crouching low enough in the closet. The reason I was banging my head against a wall not trying to crouch each time, was that the first time I crouched really low and I was still shot. I didn't make the connection that I probably left something out of place and that I need to continue to crouch regardless. I was fairly certain you didn't have to crouch very low.
I should know better after the amount of crouching I did in the first two FMV Tex games.

Personally I don't crouch at this point. I crouch in the shower because there if you don't crouch you are sure to get shot. But in the closet I never payed attention to that. What I do however is to stay away from the doors as far as the closet wall let me.

I discovered the trick about the closet at one time when I kept on getting shot , if you don't crouch down you need to be at the end of the closet that slade doesn't see you.
